The NMC SPT3 accident prevention tag is made of 8 point cardstock, is green to help convey message, with the front reading, "OK - This scaffold has been erected to meet Federal/State OSHA Standards and is safe for all craft work" with areas at bottom labeled "Date," "Competent Person," "Signature," and "Comments." This tag can be used to approve the use of a scaffold. The back of the tag reads, "Daily Inspection Record," followed by a blank grid for entering the date and a signature for inspections. It includes a 3/8" diameter hole for fastening and a reinforced brass grommet for longer wear. The tag measures 6" x 3" (H x W). (H is height, the vertical distance from lowest to highest point; W is width, the horizontal distance from left to right.)
